Among these tasks, the creation of 'nursing summaries' at patient discharge or transfer poses a particularly heavy burden. A nursing summary is a clinical document compiled from the nurse’s perspective, summarizing the patient’s course of treatment, nursing interventions, and post-discharge care needs. It plays an essential role in ensuring accurate information transfer to subsequent healthcare providers or care facilities, enabling continuous care. However, creating such summaries currently requires extracting vast amounts of data from electronic medical records (EMRs) and condensing it into concise narratives, consuming significant time.

Purpose of Implementation: Operational Efficiency and Enhanced Care Coordination at Mukojima Hospital

Under these circumstances, Mukojima Hospital identified reducing nursing summary creation time and overall documentation burden as key operational priorities to enable nurses to dedicate more time to direct patient care. The hospital also aimed to eliminate inconsistencies in documentation across staff and strengthen care coordination through standardized, high-quality records.
