"Advance Care Planning (ACP)" is an initiative to prepare for situations where one may become unable to communicate their wishes due to an incurable illness or other reasons, by discussing and sharing desired medical care and treatment with family or trusted individuals in advance. Sharing these thoughts also leads to living a rich life true to oneself. To make progress easier even when it's difficult to talk about illness and death, Yokohama City is working to promote and raise awareness of ACP by utilizing the "Moshimo Techo" (What If Handbook).
Recently, Yokohama City and the Yokohama Pharmaceutical Association concluded an agreement regarding the promotion and awareness of ACP using the "Moshimo Techo." The Yokohama Pharmaceutical Association has approximately 1,000 member pharmacies within the city and engages in community-rooted activities that support citizens, such as long-term patient support and home-visit pharmaceutical management guidance for those receiving home care. This agreement will further strengthen collaboration and promote initiatives for ACP awareness.

**Specific Initiatives under the Agreement**
* Promotion and awareness of "Advance Care Planning (ACP)" using the "Moshimo Techo" during dispensing at all member pharmacies (approximately 1,000 stores in the city), as well as at citizen-oriented lectures and events. * Implementation of training for pharmacists and sharing of utilization status and best practices, with the aim of developing human resources capable of promoting awareness among citizens.

**What is the "Moshimo Techo"?**
* The "Moshimo Techo" is a handbook that allows individuals to communicate their desired medical care and treatment by answering three simple questions (*). It is distributed free of charge, often placed in a cover that can hold a medication notebook or patient registration card, with a cumulative total of 500,000 copies distributed.
**Distribution Locations:** Ward office elderly/disabled support sections, community care plazas, home healthcare collaboration centers (home healthcare consultation rooms), etc.
