We are pleased to announce the opening of the "Nishiarai Internal Medicine Clinic" in the Nishiarai area of Adachi Ward. Starting May 1, 2026, as a primary care clinic for the community, we will seamlessly provide both outpatient and home-visit medical care, offering support for geriatric medicine, lifestyle-related diseases, and memory loss consultations, while also rolling out health support services for corporations.

Our clinic will open on the first floor of the residential nursing home "Ayumi Building 3." While catering to the medical needs of the facility's residents, we aim to be an accessible local medical clinic for people living in the neighborhood.

In addition, we will provide occupational physician services, group influenza vaccinations, and follow-ups for individuals with health checkup findings (recommendations for medical visits and online medical consultations) for business establishments in Tokyo, primarily in Toshima Ward (Ikebukuro area), Bunkyo Ward, Shinjuku Ward, and Chiyoda Ward.

Please note that we plan to hold a private viewing event on Friday, April 24, and Saturday, April 25, from 10:00 to 15:00.

Continuous Support at the Same Medical Institution, from Outpatient to Home-Visit Care

Nishiarai Internal Medicine Clinic aims to be an approachable internal medicine clinic for everyday health concerns for those living in the Nishiarai area of Adachi Ward.

In our outpatient clinic, we cover a wide range of conditions, from sudden illnesses such as colds, fevers, coughs, stomachaches, and headaches, to lifestyle-related diseases such as hypertension, dyslipidemia, diabetes, and hyperuricemia. We also handle health checkups, vaccinations, and consultations regarding health checkup results, supporting the early detection of diseases and continuous health management.

We act as an accessible internal medicine resource for consultations such as, "I don't know which department to visit," or "An abnormality was pointed out in my health checkup, but I don't know how to handle it."

From When Commuting is Possible, to After it Becomes Difficult

In aging communities, alongside managing lifestyle-related and chronic diseases, it is crucial to provide medical care for those who have difficulty commuting due to aging or changes in their medical condition.

At our clinic, we have established a system where health management is conducted via outpatient visits while the patient is able to commute, and seamlessly transitions to home-visit medical care if commuting becomes difficult. We aim to reduce the burden on patients and their families of having to find a new medical institution, ensuring they can continuously receive medical care in the community they are accustomed to living in.

In home-visit medical care, we target those who have difficulty visiting the clinic, such as local residents and nursing home residents, providing regular medical examinations, chronic disease management, medication adjustments, and consultations during changes in physical condition. For patients with a home-visit medical care contract, we maintain a 24-hour contact system and are prepared for emergency house calls when necessary.

Addressing "Memory Loss," "Insomnia," and "Medication Review" for the Elderly and Their Families

Geriatric medicine requires support not only for the diseases themselves but also encompassing the living environment, caregiving situation, and the family's anxieties.

At our clinic, in addition to general internal medicine, we handle consultations regarding memory loss, insomnia, medication reviews for those taking multiple drugs (polypharmacy management), and the appropriate timing to initiate home medical care.

We will work alongside relevant institutions to handle consultations on introducing home-visit medical care, post-discharge home recuperation, and the continuation of medical care for those who have become unable to commute.

In addition to providing medical care to local residents, Nishiarai Internal Medicine Clinic is also committed to corporate health support.

We offer the following services targeting business establishments in Tokyo, centered around Toshima Ward (Ikebukuro area), Bunkyo Ward, Shinjuku Ward, Chiyoda Ward, and Shibuya Ward:

During my 9 years in Fukushima Prefecture researching and treating the impact of aging and "loneliness/isolation" on health, I witnessed the reality that not just the disease itself, but the "loss of social connections" hinders access to medical care.

Even in this urban Nishiarai area, accompanying the aging population...
