Since the first edition in 2012, the Japan Society of Mood Disorders has continuously revised the 'Japan Society of Mood Disorders Treatment Guidelines II. Major Depressive Disorder,' incorporating the latest findings and clinical needs. However, in recent years, international standards have increasingly emphasized scientific validity, transparency, and systematic rigor in the development process of clinical practice guidelines.

In response to this context, the Japan Society of Mood Disorders has completely rebuilt the guideline from the ground up, publishing the first full revision, the 'Depression Treatment Guidelines 2025.' This publication follows the 'Minds Clinical Practice Guideline Development Manual' by the Japanese Council for Quality Health Care, systematically evaluating evidence through systematic reviews and providing recommendations based on a balanced assessment of benefits and harms.

Supporting Depression Care with a Clinically Relevant Structure

This guideline emphasizes practical clinical problem setting, aiming for a structure that aligns with real-world clinical needs. Each chapter begins with a BQ (Background Question) that poses a clinically relevant inquiry—'What should be considered when implementing this treatment?'—followed by CQs (Clinical Questions) that include summaries of systematic reviews and additional literature deemed clinically significant.

Through extensive use of charts, tables, and flowcharts, the guideline is designed to serve as a navigational map, enabling readers to align its content with their own clinical challenges and avoid confusion in the complex landscape of depression treatment.

By establishing independent chapters based on life stages such as childhood/adolescence, perinatal period, and elderly; subtypes including severity, sleep disorders, and specific diagnostic terms introduced in DSM-5; and treatment phases, the guideline achieves a higher level of individualized clinical support.

Additionally, topics that are clinically important but not suitable for systematic review formats are included as 'Topics,' anticipating future clinical application.
