Such rework, where one feels 'we have to start over again...', occurs daily in many development sites. In many cases, the background to this is a misalignment of judgment caused by 'information fragmentation'—such as changes in prerequisites or the external environment that were not visible at the time of consideration, or an inability to access knowledge that should have been accumulated within the company.

As development becomes more sophisticated and complex, how broadly and accurately one can grasp internal and external information has become a major factor in determining the quality and speed of decision-making.
