About Social Inclu Home Fuchu Wakamatsu-cho
1. Group Home with Daytime Service Support for People with Disabilities This type of group home is for individuals with severe disabilities or elderly individuals with disabilities who require continuous support during the day, allowing them to live with daily life support provided 24 hours a day, 365 days a year. (※1)
As short-stay facilities (※2) are co-located, individuals other than residents can also use them as temporary accommodation when family members are away, need a break, or in emergencies.
2. Support Tailored to Individual Needs To enable individuals with severe disabilities to lead independent lives, we provide support tailored to their disability status on a 24-hour basis.
In addition to daily life support such as bathing and toileting, meals are prepared with consideration for nutritional balance, and options like blended diets and porridge are available according to residents' needs, with meal assistance provided to those who require it. Health and medication management are conducted daily, and staff accompany residents to medical appointments and shopping. During the day, recreational activities are organized to suit residents' preferences. We also support daily financial management, thereby supporting residents' independent daily lives.
Before admission, staff with specialized qualifications will conduct interviews to understand the individual's disability status and daily assistance needs, and create an individualized support plan based on their wishes and condition. After admission, we maintain a system for promptly identifying changes in condition through continuous observation and recording of daily life, progress, and developments. Furthermore, support plans are reviewed more frequently than legally mandated cycles, allowing for flexible responses to changing needs and optimal support.
3. Operational Structure Supporting a Secure Living Environment Group homes with daytime service support have legally defined operational standards for staff deployment and facility equipment. A 24-hour monitoring and support system is required to assist the lives of those who need support day and night. Facility equipment must be barrier-free, and the living environment must be adapted to the individual's disability status.
Social Inclu Home's policy is to provide a support system that exceeds legal requirements, such as a standard of two staff members per unit (※3) during nighttime hours. (※4)
The building is newly constructed for the group home and is barrier-free, featuring accessible toilets, bathing lifts, entrance slopes, and elevators, making it suitable for wheelchair users. Security and disaster prevention equipment, including security cameras and sprinklers, are also installed.
(※1) Daytime service support is a type of shared living support (group home) for people with disabilities, established in 2018 to address the increasing severity and aging of disabilities. Shared living support is a category of disability welfare services stipulated in the Act on Comprehensive Support for Persons with Disabilities, with four types: care service comprehensive type, daytime service support type, external service utilization type, and satellite type.
(※2) Short-stay facilities are mandatory co-located facilities for daytime service support type, providing emergency temporary support for people with disabilities living in the community.
(※3) A unit refers to a living space equipped with necessary facilities (private rooms, dining/living room, kitchen, bath, toilet, washbasin, etc.) for residents. One unit accommodates 2 to 10 people.
(※4) In shared living support, the number and deployment standards for employees (staff supporting users) are determined by the number of users and their disability support classification. During nighttime and late-night hours, at least one staff member is required. Social Inclu Home generally operates with a two-person system during nighttime and late-night hours, but this may be reduced to one person depending on staffing availability.
