UnReact Launches 'Simple Contact Form | Contact Form Customizer' App for Shopify Stores

Fukuoka-based IT venture company UnReact, Inc. has released the 'Simple Contact Form | Contact Form Customizer' app.

The app was officially launched on June 2, 2026, at 10:00 AM.

'Simple Contact Form | Contact Form Customizer' is an app that allows users to easily create and install original contact forms on Shopify stores by freely combining fields such as text input, email address, radio buttons, checkboxes, and dropdowns.

Created forms can be placed on any page of the theme as an App Block, and elements such as title, fields, and button font size, color, and padding can be finely adjusted from the theme customization screen. Multiple forms can be created and managed, and placement can be switched using form IDs. Since the form submission uses Shopify's native form functionality, submitted messages are delivered directly to the store's email address.

The app can be added to themes without coding. Pricing starts at the Basic Plan for $9.99/month, with a 7-day free trial available. Paying annually offers a discount equivalent to two months free.
