The Culmination of Coaching Practiced by Physicians of the Japan Society of Diabetes and Healthcare Who Empathize with Patients
In diabetes care, it is essential for people with diabetes (PwD) to actively engage in their treatment for successful outcomes. However, in chronic disease management, the difficulty of maintaining long-term adherence often stands as a barrier for healthcare providers. The Japan Society of Diabetes and Healthcare, which deeply considers the psychology and behavior of PwD, has long discussed and accumulated experience on how appropriate use of coaching techniques can overcome this challenge. This book presents the culmination of those efforts, delivered to physicians and other healthcare professionals.
Insights to Address the Challenges Faced by Primary Care Physicians in Diabetes Care
(Adapted from the Preface)
Primary care physicians often face challenges such as the following:
- When blood glucose or HbA1c levels are high, I ask about diet, exercise, and snacking, but I realize I sound like I'm taking a police statement, leading to self-loathing.
- When I listen empathetically to patients, I sympathize with their circumstances—such as 'I ate because I was stressed' or 'I don’t have time to eat slowly and chew properly'—and end up saying, 'Well, I guess it can’t be helped. Try again next time,' postponing meaningful action.
- When I suggest nutritional guidance again, I’m told, 'I already understand well enough, thank you.' Why is this? Is nutritional guidance perceived as a 'punishment game'?
- Occasionally, patients transfer to me despite me not being a specialist. When I ask why, they say, 'The previous diabetes specialist was too strict and always scolded me, so I couldn’t continue. I heard you’re not someone who gets angry easily…' This leaves me with mixed feelings.
This book contains insights to address such concerns. Diabetes care is not solely the responsibility of diabetes specialists or diabetes care teams. It is a lifelong commitment carried out by trusted primary care physicians, general practitioners, and family medicine specialists in their respective communities.
How do we build and sustain human relationships to walk alongside PwD over the long term? This book serves as a valuable resource for learning how.
