## Two Observational Studies from Kokura Memorial Hospital at WCN 2026

The following studies were presented:

- "Development of a Peritoneal Dialysis Program through Collaboration between Nephrologists and Primary Care Physicians: Single-Center Experience" (WCN26-AB-5416)
- "Structure and Economics of a Collaborative Care Model between Nephrologists and Primary Care Physicians in Peritoneal Dialysis: Single-Center Experience" (WCN26-AB-5850)

These reports document co-management efforts between primary care physicians and nephrologists at a single facility.

## Chronic Kidney Disease and Renal Replacement Therapy in Japan

In Japan, approximately 15 million people, or one in eight adults, are estimated to have Chronic Kidney Disease (CKD). When CKD progresses to end-stage renal failure, renal replacement therapy options include peritoneal dialysis (home dialysis), hemodialysis (clinic-based), and kidney transplantation.

While peritoneal dialysis prevalence is 12–70% in other countries, it remains low in Japan at approximately 3%, with limited medical institutions capable of providing it.

## Research Details and Implications

The study on structure and economics demonstrated that Kokura Memorial Hospital expanded its peritoneal dialysis patient base to 320, becoming one of Japan's leading centers, by collaborating with primary care physicians. As a home-based treatment, peritoneal dialysis requires no new facility investments or expansion.

The study on program development reported that a regional linkage system shifted the peak age of dialysis initiation from the 60s to the 70s.

As of the end of 2023, there were 343,508 dialysis patients in Japan, with an average initiation age of 71.59 years.
