Background of Development
─ "Staff Shortages" and "Digital Fatigue" in Medical Settings
Against the backdrop of the "Work Style Reform for Physicians" that came into effect in April 2024, medical institutions face the increasingly critical challenge of balancing the quality of medical care with the sustainability of operations with limited staff. As recruitment becomes more difficult and recruitment costs through agencies rise, there is a growing need to create operational structures that do not rely solely on increasing personnel.
Meanwhile, medical settings often coexist with electronic health records, departmental systems, paper, fax, and internal phone systems (PHS). The proliferation of systems and tools for different purposes has led to an increase in things to learn, search for, and confirm. This results in delays in accessing necessary information, redundant data entry, reliance on individuals for handovers, and interruptions for confirmation. Furthermore, many existing IT solutions are designed for PC-centric office workers, and work environments optimized for essential workers, including healthcare professionals who work while on the move, are not yet fully established.
Addressing these structural challenges in medical settings, kotomos has been developed as a new operations support platform utilizing smartphones and AI. It makes everyday communication, information sharing, and task confirmation easier to handle on smartphones, and by combining this with AI-powered search of internal hospital information and support for clinical decision-making, it provides a work environment that accommodates the mobile and frequently interrupted nature of work in these settings.

Enables real-time information sharing across departments and job roles. Hospital-wide notifications can be delivered to targeted recipients, and features like read status tracking and reminders for unread messages support reliable dissemination of information.
Inter-Facility Collaboration
Facilitates communication with collaborating medical institutions and care facilities through dedicated channels, supporting patient transfers, discharge support, and regional medical collaboration across facilities.
Contact Management
Centrally manages contact information for hospital staff, collaborating facilities, and business partners, enabling quick access to necessary contacts.
Calendar / Shift Management
Provides centralized management of schedules such as outpatient appointments, surgeries, rounds, and meetings, allowing for tracking by person in charge or team. Shift management supports everything from collecting work requests to publishing confirmed shifts, notifying changes, and generating reports.
File Management
Centrally manages internal hospital documents, regulations, and manuals, supporting secure access to the latest information through version control and access restrictions.
Patient Information
Allows reference to patient-specific notes, instructions, and shared information from mobile devices, creating an environment where necessary information is easily accessible at the bedside or while on the move.
Fax Integration
Enables viewing of incoming faxes, sending, assigning to responsible staff, and managing response status all within kotomos. It reduces the need for paper sorting and internal relay, supporting confirmation and response from smartphones.

AI assists with tasks such as researching, summarizing, and confirming information.
Clinical Support AI
Provides information based on medical literature in response to questions about diagnosis, treatment, and tests, with sources cited. It supports healthcare professionals in information verification and aids daily clinical decision-making.
Internal Knowledge AI (In Development)
AI reads internal documents, regulations, and manuals stored in file management to search and extract necessary information. It supports natural language queries and also assists with operations within kotomos, such as chat and schedule registration.

Operates under an information management system compliant with three ministries' guidelines and aiming for ISMS certification, supporting authentication, access control, encryption, and audit logs. Ensures the safety and traceability required for a business tool handling medical information.

Provides extensibility for easy integration into existing systems used by medical institutions, aiming to reduce information fragmentation and connect entire business workflows.
Pilot Program Recruitment Overview
We are currently recruiting pilot medical institutions willing to provide feedback on the product and its actual use. While primarily targeting hospitals, we also welcome inquiries from clinics, nursing care facilities, home healthcare-related facilities, and other medical and care settings facing challenges in information sharing and operational collaboration.
