Each omikuji includes:

About Tomiyashouoten
Based in Tomigaya, Shibuya-ku, Tokyo, Tomiyashouoten launched 'Saxe Bleu Diner,' a roast beef specialty restaurant curated by Yoyogi Imahan, in 2014.
