In recent years, with the advancement of medical DX, the utilization of generative AI in the medical field has rapidly expanded. Its practical application as a technology contributing to reducing the workload of medical professionals and improving the quality of medical care, such as creating and summarizing medical records and generating patient explanations, is accelerating. It is expected that generative AI will become indispensable in medical operations in the future.
On the other hand, as cyberattacks become more sophisticated and cunning, security measures accompanying the use of generative AI are not necessarily sufficiently established.
For example, in the FY2026 medical fee revision, new evaluations for the utilization of generative AI and robots were positioned, and the importance of appropriate cybersecurity measures was indicated as a prerequisite.
However, cyber risks in generative AI have many qualitatively different characteristics from the cyber risks assumed by conventional medical information systems. While conventional medical information systems primarily aim to store, refer to, and share input data, generative AI's main purpose is to generate new texts and judgment support information based on input information while connected to external services and learning platforms, thereby supporting operations.
Therefore, the design of medical operations using generative AI itself is thought to inherently carry the risk of altering information distribution channels and decision-making processes, creating new points of impact for attacks and failures.
However, in many medical institutions, due to the continuation of traditional measures or problems such as a lack of human resources, it is difficult to say that the necessity of security measures based on these new risks has been sufficiently considered and appropriate measures have been taken.
Given this situation, the Medical AI Platform Collaborative Innovation Partnership (HAIP) and the General Incorporated Association Health ISAC Japan (HIJ) have decided to issue a joint proposal on the importance of security for realizing the safe and effective utilization of generative AI in the medical field.
This proposal highlights the importance of planned initiatives for individual medical institutions to gradually establish optimal security governance led by management, such as chairpersons, hospital directors, and administrative managers. Furthermore, from a medium-to-long-term perspective, based on the idea that a mechanism is needed to prevent disparities in security responses among medical institutions due to differences in resources from affecting the quality of original medical operations, the proposal raises the necessity of a joint platform to realize continuous and equitable safe utilization of generative AI.
HAIP and HIJ will, in the future, collaborate with medical institutions and stakeholders from government, industry, and academia to concretize this proposal, jointly promoting guideline development, strengthening information sharing infrastructure, human resource development, and demonstration/dissemination activities.
Through these activities, we aim to balance the utilization of generative AI in the medical field with appropriate cybersecurity, contributing to patient protection and the realization of a sustainable medical care provision system.
